Defects of materials can result in significant damages in the operation. They can lead to a complete collapse and even life hazard. For this reason, it is necessary to look for the causes and take measures, to minimize the defects.

FA – Failure Analysis is a discipline whose output is the analysis based on the tests and findings for presented samples, or on-site inspections.

RCA – Root Cause Analysis is a discipline that allows evaluation of possible causes that resulted in the tested defect or failure.

The purpose of the process is to prevent failures, and it can include an assessment of the suitability of design, material, operating parameters etc.
